Mechanism of Action
hCG is one of the few compounds in this catalogue whose mechanism is established rather than proposed: it has a solved structure, a defined receptor and approved clinical uses. What follows describes that pharmacology, which belongs to characterised prescription material and not to a research-grade vial of unverified identity.
Molecular structure. X-ray crystallography of human chorionic gonadotropin showed a heterodimer in which both subunits adopt a cystine-knot fold, and that the paired dimer, not either chain alone, is the active species. The beta subunit carries a C-terminal extension with O-linked sugars that LH does not have, which slows renal clearance. Circulating hCG is also not a single molecule — hyperglycosylated forms, free beta subunit and nicked variants coexist — so an assay reporting "hCG" does not by itself establish which isoform is present.
Receptor target. hCG acts at the luteinizing hormone/choriogonadotropin receptor (LHCGR), a G-protein-coupled receptor it shares with LH. Work in transfected cell lines and in human granulosa cells indicates the two ligands are not interchangeable at that shared receptor: they differ in occupancy time and appear to bias intracellular signalling differently.
Downstream signalling. Receptor activation couples through Gs to adenylyl cyclase, raising cAMP and activating protein kinase A, which drives StAR-mediated cholesterol transport into mitochondria and so steroidogenesis. In the ovary this sustains progesterone output from the corpus luteum; in testicular Leydig cells the same cascade drives testosterone synthesis. This comes from cell-culture and animal steroidogenesis work alongside human clinical pharmacology.
Approved clinical context. Chorionic gonadotropin (urinary-derived) and choriogonadotropin alfa (recombinant) are both approved drugs, but their labelled uses differ: the recombinant product is approved to trigger final follicular maturation and ovulation in assisted reproduction, while the urinary-derived product also carries approvals in prepubertal cryptorchidism and male hypogonadotropic hypogonadism. Those approvals attach to specific manufactured products and their labelling.
Where the human evidence is explicitly negative. US regulation requires hCG drug labelling to state that hCG has not been demonstrated to be effective adjunctive therapy in the treatment of obesity, and that there is no substantial evidence it increases weight loss beyond that resulting from caloric restriction, alters fat distribution, or reduces the hunger associated with calorie-restricted diets. This is the one widely promoted context in which the human data were tested and did not support the claim.
